Notice is hereby given that a hearing will be held at 10:00 AM on August 11, 2004, at the offices of the Alabama Department of Insurance, 201 Monroe Street, Suite 1700, Montgomery, Alabama, as follows:

 

Revision of Rule 482-1-071-.15, so as to delete paragraph F regarding rate structure on attained age greater than one year.  (3 pages) 

 

Revision of Rule 482-1-079-.03, so as to delete the former address of the Alabama Department of Public Health Bureau of Licensure and Certification.  (1 page) 

 

Revision of Rule 482-1-079-.07, so as to replace the former cite of Chapter 8 with 7, the new producer licensing law.  (1 page) 

 

Revision of Chapter 482-1-081, Licensing of Sales Producers for Health Maintenance Organizations, so as to provide for the licensing of sales producers for Health Maintenance Organizations.  (5 pages) 

 

Revision of Rule 482-1-133-.09, so as to add a six percent (6%) interest rate for violations on amounts refunded in cash.  (1 page) 

 

Adoption of Preneed Regulation No. 1 [Chapter 482-3-001], which provides standards for the sale of preneed funeral and cemetery merchandise and services and defines the accounting and record keeping requirements for persons holding preneed certificates of authority under the Alabama Preneed Funeral and Cemetery Act of 2002. (17 pages)

 

Any person wishing to submit testimony at said hearing is requested to submit said testimony in writing to the Alabama Department of Insurance, Attention: Legal Division, Post Office Box 303351, Montgomery, Alabama 36130-3351.  Any written submission of testimony prior to the hearing will not preclude anyone from making an oral or written presentation at the hearing.
